Do you want to become a donor? Or not?

All people of age twelve and above residing in the Netherlands can have their wishes regarding organ and tissue donation registered in the Donorregister. Registration provides clarity and certainty for everyone involved. Registering your wishes can only be done by using the donor form. This form is only available in Dutch. You can choose from four different options:

Option 1 Yes, I give permission
Option 2 No, I don't give permission
Option 3 My next of kin decide
Option 4 A specific person decides
