Options

You have four options when registering your decision in the Donorregister:

Option 1 Yes, I give permission

I want my organs and tissues to be used for transplantation after my decease.

The organs and tissues which can be transplanted, are mentioned on the donor form. You can mark the organs and/or tissues you do not want to donate.

Option 2 No, I do not give permission

I do not want my organs and tissues to be used for transplantation after my decease.

Option 3 My next of kin decide

I wish to leave the decision to my family and possible partner. My next of kin decide whether my organs and tissues may be used for transplantation. Your next of kin are (in this order):

  • the husband/wife or partner with whom you live at the time of your decease
  • If he/she cannot be reached immediately:
    your adult blood relations up to the second-degree (parents, grandparents, children, grandchildren, brothers and sisters)
  • If they cannot be reached immediately:
    your adult second-degree relations by marriage (parents or a brother or sister of your husband/wife or partner)

Option 4 A specific person decides

I am leaving the decision to someone else. I am authorizing this person to make the decision after my decease. He or she decides whether my organs and tissues may be used for transplantation.

If this person cannot be contacted at the time of your death, the decision will be left to your next of kin.

Please note: if the personal details of this particular person change or you wish to specify another person, you must notify the Donorregister yourself by filling in a new donor form.
