Removal from the Donorregister

If you wish to have your registration removed from the Donorregister's database, you have to complete the form Verwijdering uit het Donorregister (Removal from Donorregister). The form is only available in Dutch.

You will receive a written confirmation of the removal of your registration. Having been removed from the Donorregister's database results in not having a recorded decision in the Donorregister. In that case the Dutch Organ Donor Act requires that your next of kin are to be asked whether or not you would have wished to be a donor.
